Back Conditions We Treat in Boston
Lower back pain
Lower back pain is the number one reason people walk through our door. Whether it started after deadlifts, a long flight, picking up a kid, or seemingly out of nowhere - we'll help you understand what's going on and build a clear plan to get out of pain and stay out.
Sciatica
That sharp, shooting, sometimes burning pain/numbness/tingling that runs from your low back down your leg. Sciatica is often misunderstood and poorly managed. We'll assess what's actually driving your symptoms and give you active strategies to feel better and get back to valued activities.
Disc herniations/Bulges
A disc herniation isn't a life sentence and it doesn't necessarily mean you need surgery or invasive interventions. The vast majority resolve with conservative care, and our integrated approach has helped many active Bostonians return to running, lifting, and life.
SI joint pain
Often described as a deep, aching pain just off to the side of the lower back/sacrum - common in pregnancy, postpartum, and athletes.
Lower back strains and sprains
Acute injuries from a random twist, heavy lift, or sport collision. We'll help you settle things down and gradually re-load the area, and return to your activity stronger than before.
Pregnancy and postpartum back pain
Up to 70% of pregnant women experience back pain. We work with pregnant runners, lifters, yogis, and active moms - and brand new parents - to help them stay active, comfortable, and confident through every stage.
Post-surgical back recovery
Recovering from a discectomy, fusion, or other spine surgery? We integrate with your surgical team's plan to guide you through progressive rehab and restore strength, movement, and confidence.
Chronic back pain
If you've been dealing with back pain for months or years and haven't made meaningful progress, you're not broken. Chronic back pain is complex but absolutely manageable. We use the most current pain science to help you understand what's driving your symptoms and modern exercise and rehab science to rebuild your relationship with movement.
When Should You See Someone for Back Pain?
A good rule of thumb: if back pain is interfering with your sleep, work, training, or daily life — and it's been more than a few days — it's time to get evaluated. Don't fall into the trap of "wait and see" turning into months of guarded movement, fear, and deconditioning.
Some signs you should be seen sooner rather than later:
Pain that radiates down a leg
Numbness, tingling, or weakness in your legs
Pain that's worse at night
Pain following a fall, accident, or significant injury
Pain that hasn't improved after 2–3 weeks of self-care
Red flags - go to the ER: loss of bladder or bowel control, sudden severe weakness in the legs, or significant numbness in the saddle/groin region.
For most cases, a short visit can save you weeks of unnecessary suffering and put you on a clear path forward.
Our Approach: What Actually Works for Back Pain
The back pain industry is full of gimmicks - lasers, decompression tables, magic-alignment promises, long-term adjustment plans. Most of it isn't supported by good evidence. Here's what we actually use, all backed by current research:
Movement-based rehabilitation. The single most important component of recovery. We don't hand you a sheet of exercises - we coach you through movement, build a progressive program, and adjust as you improve.
Manual therapy. Hands-on techniques including joint manipulation, mobilizations, and soft tissue work - used strategically alongside active care to reduce pain and improve movement quality. Manual therapy alone is rarely enough; combined with active rehab, it accelerates progress.
Mindfulness and breathing. Pain has both a physical and mental component. Targeted breathing and mindfulness work helps calm an overactive pain response - especially valuable for chronic or stubborn cases.
Progressive strength and conditioning. Strong backs are resilient backs. We blend rehab with strength training so you don't just get out of pain - you come out stronger than before the injury.
Pain education. Understanding your pain reduces fear, improves outcomes, and changes the way your nervous system processes signals. We'll explain what's actually going on (and what's NOT going on) and help you get back to life with confidence.
Client Reviews
“I’ve been working with Tim for a few months. I came to BBH in a really bad spot. I was in a lot of back pain and had no strategy to get myself out of it. Tim has been a pleasure to work with and has helped me build a toolkit that has meaningfully improved the quality of my life. I’ve since referred my dad. It is not convenient for me to get to back bay but I wouldn’t go anywhere else.”

“I was having some pregnancy pain both during work and my workouts. I am a physical therapist so being able to keep myself moving and lifting was not only important for me personally but necessary for my job. Laura was able to help give me modifications for exercises and helpful stretches and movements as pregnancy progressed which allowed me to not only work all the way up until my delivery but keep my workouts until 38 weeks!

“I am a former D3 athlete and now "retired" adult who prefers to stay active but has unfortunately dealt with ankle, hip, and back injuries the last six years. I turned to Tim and Laura about a year and a half ago and they have been tremendous. They made sure to incorporate my goals, what I like to do to get exercise, and my general feelings about my body and activity levels into my recovery. I could not recommend them more!”

Your Questions, Answered
-
For most back pain, the approach matters more than the credential. We integrate the best of both worlds - skilled manual therapy plus movement and strength-based rehab - into a single plan. The right provider is one taking an evidence-based approach and matching their care to your goals.
-
Almost never. Research shows that early imaging for non-specific back pain often leads to over-treatment and worse outcomes. We'll assess you carefully and recommend imaging only if there's a clinical reason to.
-
Yes. We regularly work with people post-discectomy, post-fusion, and post-laminectomy. We coordinate with your surgical team when needed and help you progress safely through every stage of rehab.
-
No. We focus on active strategies and education that are supported by evidence. We're not opposed to adjuncts when they're appropriate, but our care is centered on movement, strength, manual therapy, and education.
-
Almost always, yes. A core part of our approach is keeping you moving - modifying training rather than shutting it down. Complete rest is rarely the answer for back pain.
-
It depends. Many people can see meaningful change within the first 4–6 visits, with a typical care plan ranging from 6 to 12 visits depending on complexity. We'll give you a realistic estimate after your first visit.
